Childcare Forms

Does your school, daycare, camp or other activity need a form for you or your child?

There is no charge for a form if it is completed during your well visit. Each additional form will incur a $10 fee that is payable when the form is requested. The turn around time is typically 3-7 business days. An urgent form fee of $20 can be paid if a form is needed within 2 business days and our staff is able to accomodate the request. 

Please complete the parent portion of your form and complete the Activity Update. Forward both to your Patient Portal Account and someone will contact you when the form has been reviewed or completed.
